The presentation will cover how the CSB conducts forensic science investigations of catastrophic events within industrial settings; what CSB looks for in determining root causes; and the circumstances surrounding the catastrophic events of West, TX fertilizer plant, Chevron refinery fire in Richmond, CA, and the Deepwater Horizon.
