  1. Will it be necessary for me to file a paper copy of the Quarterly Report of Wages Paid?

    No, an employer utilizing the EFT System will not need to file a paper report as well. The ccd+ addendum record contains required fields for Total Wages and Taxable Wages. This data uploads to the employer account during the electronic transfer.

  2. How do I submit my Quarterly Wage Report?

    Employers can report their payroll wage information by magnetic media or by filing the paper report of the Employer's Wage Report portion of the LDOL ES-4.   3. Can I file my Quarterly Report of Wages Paid using the EFT System without submitting an EFT Payment?

    Yes, an employer who has zero total wages and/or taxable wages can use the EFT System.

    Yes, cents must be reported, even if zero.

  5. What happens when I return the Authorization Agreement Form to LDOL?

    Upon receiving the employer's Authorization Agreement Form, the employer will be sent a letter of acceptance verifying their enrollment in the EFT System. Use this link to download this form (insert link for EFT Authorization Agreement).

    Yes. The fax number is (225) 342-2955 and is also found on the form.

    Yes. Call UI Accounting at (225) 342-2994 or fax information to (225) 342-5844.

  9. How do I reflect or handle a Notice of Overpayment?

    If an employer has recently received a Notice of Overpayment, the overpayment amount can be subtracted from the computed tax due prior to submitting the EFT Payment.

    This option is currently not available. Contact LDOL at (225) 342-2961 to request an Amended Report Form LDOL ES-51 or download the Quarterly Wage & Tax Report -Amendment LDOL ES-51 (insert line for Quarterly Wage & Tax Amendment Form).
